1579 BELLEVUE HOME PROFILE
Location: 1579 Bellevue, Sudbury,Ontario
Type of Home:
Staff operated male adolescent program for 7 youth in need of social, emotional and behavioural skills development for daily living and to prepare for independent living.
Physical Description:
This very large 4 story split level home has 7 bedrooms, a staff office, 3.5 bathrooms, one large kitchen and dining room, a large living room, a sitting area, a laundry area, a play room, a covered deck in the back facing the partially fenced in yard and parking for 4 vehicles.
Curriculum Vitae:
All the staff have completed Summit’s Orientation training, Therapeutic Crisis Intervention Training and certification, job shadowing and coaching from the Team Leader, also on-line courses are mandatory, addressing the client population we serve. All staff are also certified in First Aid and CPR as well as having provided their police checks and vulnerable sector checks. On going courses and workshops are offered regularly for all staff to further develop their skill set.
Child Care Support:
Residential Youth Workers work 8 hours shifts based on the occupancy rate of the home, the program’s schedule and appointments for the day. The staff/youth ratio is 3-7 based on full occupancy rate. There is a full-time Team Leader on site for case management and supervision.
Placement Preference:
Male teens in need of a structured home environment with an emphasis on positive community activities, developing a healthy self-esteem, learning social skills and life skills, and building healthy relationships would benefit from this home. For socially and emotionally challenged teenagers who can function within a community based environment. We are not able to provide care for Young Offenders serving custody or detention orders. We are also unable to provide care for known sexual offenders, fire setters, or violent teens; however each case will be assessed individually.
Recreation Facilities and Activities:
The home is near Moonlight Beach, Carmicheal Arena, a skateboard park, Minnow lake for fishing, across the street from a pharmacy and close by shopping areas. The home is also a short walk from the local movie theatre, and medical/dental/optical services. The home also utilizes community recreational facilities such as the YMCA, attending Wolves games and participating seasonal festivals and activities. The home also has access to the following school: Pius XII, Adamsdale P.S., Churchill P.S., Sudbury Secondary, Re-Start Program, and the Catholic school alternative program.
General Overview:
Well structured, reality based home capable of handling teens with emotional, behavioural and social problems. Positive social skills, self-regulation, and making better choices are all emphasized daily through staff supervision and coaching as well as the promotion of volunteer work in the community.

13 ELMHURST HOME PROFILE
Location: 13 Elmhurst, Sudbury,Ontario

Staff operated female adolescent program for 6 teens between in need of social, emotional and behavioural skills development for daily living and to prepare for independent living.
Physical Description:
This large 3 story split level home has 6 bedrooms, a staff office, 1.5 bathrooms, a kitchen and dining room, a living room, finished rec.room in the basement, a laundry area, a back yard deck and shed, a partially fenced in yard and parking for 4 vehicles.
Curriculum Vitae:
All staff have completed their post secondary studies in a human services field such as: Child and Youth Worker program, Social Services program, Addictions Counselling program, Native Studies program. The team is composed of female staff that have completed Summit’s Orientation training, Therapeutic Crisis Intervention Training and certification, job shadowing and coaching from the Team Leader, also on-line courses are mandatory, addressing the client population we serve.
